Pondera Home Health Agency provides coordinated and comprehensive skilled care to patients of all ages in their home.  Specific treatment and services rendered are individualized under the direction of the patient's physician.  Home health care is available to home-bound patients that are recovering from an illness, injury, surgical procedure or hospitalization. It is also available for patients whose level of wellness has deteriorated in the home setting.  PMC provides the below services in Home Health. 

NEW!!!  The Home Maker Program  NEW!!! 

Available to Pondera County residents that are 60 years of age or older. Qualified residents can use the following services:  

  • Homemaker Services: light housekeeping, laundry, dishwashing, and grocery shopping

  • Respite Care: provides brief period of relief or rest for caregivers. 

Nursing - Skilled Nursing Services

A Registered Nurse may visit the patient's home to make an assessment of all body systems, blood drawing for laboratory tests, wound care, blood sugar monitoring, injections, IV therapy, medication monitoring, and teaching to patient and family.  A Registered Nurse is available 24 hours a day including weekends and holidays to answer questions and address patient concerns.

 

Rehabilitation Services

A licensed therapist may visit the patient's home for a variety of treatments in the below rehabilitation services. 

Physical Therapy 

Treatments are used to reduce pain, restore function, increase mobility and strength. 
Individualized mobilization programs created by our team to help the patient reach their goals and overcome the challenges they face. Our Physical Therapists evaluate and provide one-on-one, evidenced based treatment for low back pain, neck pain, headaches, arthritis, osteoporosis, post-surgical joint replacements, sports injuries, pelvic pain, incontinence, developmental delays and many other diseases and conditions.  We will educate the patient and their family and give them the tools needed to:

  • Prevent Disability

  • Ease Pain

  • Restore Function

  • Increase Mobility

  • Improve Quality of Life
     

Speech Therapy

Improve communication, & treat speech, swallowing, & cognitive disorders. 
Our licensed Speech-Language Pathologist provides treatment of speech disorders, language disorders, verbal and non-verbal communication, cognitive handicaps, Autism, Dementia, Parkinson. We will educate the patient and their family and provide:

  • Cognitive Therapy

  • Dysphagia Evaluation and Management

  • Team Approach to Parkinson’s treatment (LSVT Certified)

 

Occupational Therapy

Improve ability to perform daily activities, restore physical function and independence. 

Occupational Therapists provide specialized hand therapy for post-trauma, post-surgical and chronic conditions, such as arthritis; fibromyalgia and repetitive stress injuries, such as carpal tunnel syndrome; sports injuries; acute medical and neurological conditions.  We will educate the patient and their family on ways to:

  • Perform activities of daily living (ADL) in a safe and simplified manner

  • Modify work-place ergonomics

  • Complete home safety evaluations

  • Provide caregiver and family education & instruction

  • Complete neuromuscular re-education, therapeutic exercise & splinting.

Insurance Coverage

PMC Home Health Agency accepts assignment from most insurance.  Contact your insurance company to discuss your benefits.  Co-pays and deductibles are applicable. We are available to discuss Medicare criteria and eligibility with interested clients.
