Pondera Medical Center's Extended Care Facility serves as a home to many of the founders and builders of this community. Surrounded by beautiful golden grain fields and in the shadow of the Rocky Mountain front, our facility is a wonderful place to call "home" later in life. Our residents are well respected, cared for and treasured.

We are a 59-bed nursing home offering 24-hour care and nursing services provided by licensed, certified, and compassionate staff members. The facility itself is beautiful, clean, and bright.  The Extended Care has 37 private rooms, 11 spacious semi-private rooms, with an additional 14-bed Special Care Unit designated for residents with advanced memory loss and confusion.   Specialized services are also available including those offered by a Physical Therapist, Occupational Therapist, Speech Therapist, Registered Dietitian, and a Social Worker. 

 

We have a remarkable Life Enrichment Program which follows the philosophy of the ten Eden Alternative principles. Activities are offered seven days a week and include exercise, socials, games, theme parties, pets, gardening, cooking, live music, crafts and outings.

 

The variety of food prepared in our aroma-filled kitchen is nutritious, hearty, and flavorful. The dietary staff offer alternatives and choices for each meal and snack. Residents and families can provide their favorite recipes from home which our Registered Dietitian includes in monthly meal choices.

 

Our nurses and nursing assistants are extra-special! Many have worked here for years and their longevity speaks for itself.
